After an incident in prison, the focus is often on finding who was at fault. Instead, we asked ourselves: how can we learn from incidents together, as a team?
The Incident Box is a way to change behaviour in a positive way. With the concept of The Golden Half Hour, teams have an effective instrument to help them reflect immediately after an incident: they can look back, learn lessons, and plan ahead. Tools like a kitchen timer and question cards make it easy to do so. The goal? Less placing blame, more learning together, and preventing future mistakes.
